  19.         paper writing thereunto annexed and marked as Exhibit "A"

  22.         and for his/her last will and testament, by signing 
  23.         his/her name at the foot or end thereof, and that at the 
  24.         time of the execution of the will the testator was of the 
  25.         full age of majority, and, in my opinion, the testator 
  26.         was of sound mind, and understanding at the time of the 
  27.         execution of the will.

  29.         3.      That the will was so executed by the testator in 
  30.         my presence, and in the presence of ______________________

  32.         time; whereupon the said other witness and your deponent, 
  33.         did, in the presence of the testator, attest and subscribe

  59.  
  60.         -this affidavit has many blanks, and I have not noted all of same 
  61. in the left margin.

  63.         -use also where the testator signed with a mark or was blind:
  64.         "That previously to the execution of the will by the testator, the 
  65. will was read over to the testator by me (or by the person who did so read in 
  66. your presence), and the testator at that time had a knowledge of its contents 
  67. and appeared perfectly to understand the will.
  68.  
  69.         -use where the will was signed by a third party for and on behalf of 
  70. the testator at this request, change paragraph #2. to read, in part:
  71.         "by _________________________ signing the name of the testator to the 
  72. will at the request of, and in the presence of, the testator, who was 
  73. physically unable to sign his/her name or make his/her mark."

  75.         -this affidavit can be used for a codicil, but change the work 'will' 
  76. to 'codicil" where found.

  78.         -this affidavit can also be used after the death of the testator, if 
  79. you change the word "know' in paragraph 1. to "knew'; and also add the word 
  80. "deceased" at the end of paragraph 1.
